Mrs. Viva Gaye Formby, 68, of Ozark, died Wednesday, April 8, 2009, in a Dothan hospital following a short illness. Funeral services will be 11:00 AM Monday, April 13, 2009, from Holman Funeral Home Chapel with the Reverend Isom Hill officiating, interment will follow in Camp Ground Baptist Church Cemetery, Holman Funeral Home of Ozark directing. The family will receive friends at the funeral home from 4:00 until 6:00 PM Saturday, April 11, 2009. Mrs. Formby was born January 21, 1941 in Ozark to the late Bernice Frances Thompson and Ray Vernon Turner. She was a graduate of Saint Margarets School of Nursing in Montgomery and worked at the hospital for several years and was the director and founder of the Super Tuesday Day Care program at Cloverdale Baptist Church in Montgomery. Mrs. Formby returned to Ozark in 1987, working with Ozark Health and Rehab and Oakview Manor until her retirement. She was a member of Cloverdale Baptist Church in Montgomery. She was preceded in death by her sister, Linda Turner. Survivors include one son, John Larkin Formby, Jr. and his wife Heather, Montgomery. One sister, Pam Turner, Ozark; two brothers, David Ward and Sammy Turner both of Ozark. One Grandson, Cole Formby. One niece, Jenifer Ward and Nephew, Josh Ward. Flowers will be accepted or memorials may be made to Easter Seals of Alabama, 5960 East Shirley Lane, Montgomery, Alabama 36117.
